1. GENERAL TERMS OF SERVICE 

  • New clients may contact Mindbodyology Inc. at 1-833-444-MIND. Current clients should contact the administrative team through their client portal.
  • To cancel an appointment, current clients should use the client portal at least 48 hours before the scheduled appointment to cancel it themselves. Clients may also contact the administrative team for assistance with cancellation through their client portal also with 48 hours’ notice (in writing via the client portal only). Late cancellations and missed appointments may be subject to a $110 fee.

 

2. SMS MESSAGING TERMS 

  • By opting in, you agree to receive SMS messages from Mindbodyology Inc. regarding appointment confirmations, appointment reminders, copay information, and customer-care communications through the client portal.
  • Message frequency may vary. Message and data rates may apply. Reply STOP to opt out and HELP for assistance. For additional support, contact Mindbodyology Inc. through the support contact listed on this website.
  • Your consent to receive text messages is not required to purchase services. You may opt out at any time by replying STOP. After opting out, you may receive one final message confirming your request. You may opt in again by following the available enrollment instructions.
  • Mindbodyology Inc. may update these SMS messaging terms when necessary. The Terms of Service and Privacy Policy available on this website govern your use of our services. SMS opt-in information and mobile information will not be shared with third parties for their own marketing purposes.

 

3. FOR NEW CLIENTS

  • Please call or email to request an appointment time at 1-833-44-MIND and/or info@mindbodyology.org , before registering on client portal.
  • After speaking with us and receiving your appointment confirmation, please complete the forms listed below at least 48 hours prior to your appointment time.
  • You will receive a “Welcome Email” that will include a direct link to the client portal, and a username and passcode.  Please use these to register onto the Mindbodyology portal, or simply click on “Client Portal” in the menu above to begin the process.
  • The MINDBODYOLOGY™ Office Admin team will be notified of your registration and completion of required paperwork automatically.

5. ADDITIONAL TERMS OF SERVICE

  • All clients are required to place a credit/debit card on file to cover service fees, co-pays, deductibles and/or missed appointments.
  • Cancellation Policy: Clients must cancel/reschedule with at least 48 hours of notice.
  • All clients with insurance must also input insurance information under “Insurance” tab in menu in the client portal prior to the first appointment.
  • All clients will be matched to a Mindbodyologist therapist according to specialty and client-therapist fit.
