Terms of Service

1. FOR NEW CLIENTS

  • Please call or email to request an appointment time at 1-833-44-MIND and/or info@mindbodyology.org , before registering on client portal.
  • After speaking with us and receiving your appointment confirmation, please complete the forms listed below at least 48 hours prior to your appointment time.
  • You will receive a “Welcome Email” that will include a direct link to the client portal, and a username and passcode.  Please use these to register onto the Mindbodyology portal, or simply click on “Client Portal” in the menu above to begin the process.
  • The MINDBODYOLOGY™ Office Admin team will be notified of your registration and completion of required paperwork automatically.

  • Biopsychosocial

  • Consent to Touch Therapy & Energy/Reiki Treatments
  • Client Understanding & Agreement Form

 

3. ADDITIONAL TERMS OF SERVICE

  • All clients are required to place a credit/debit card on file to cover service fees, co-pays, deductibles and/or missed appointments.
  • Cancellation Policy: Clients must cancel/reschedule with at least 48 hours of notice.
  • All clients with insurance must also input insurance information under “Insurance” tab in menu in the client portal prior to the first appointment.
  • All clients will be matched to a Mindbodyologist therapist according to specialty and client-therapist fit.
